Behaviour Assessment Report (BAR)

Functional Behavioural Assessment (FBA)

The main goal of an BAR/FBA is to:

  • Understand what triggers the behaviour (antecedents)
  • Describe what the behaviour looks like (behavioural description)
  • Identify what follows the behaviour that might be reinforcing it (consequences)
  • Determine the underlying need or purpose (the “function”) — e.g., gaining attention, escaping a task, accessing a preferred item, or sensory regulation

Development of Behaviour Support Plan (BSP)

The main goal of a BSP is to:

  • Implement proactive strategies to prevent behaviours of concern (prevention and environment modification)
  • Teach new, positive skills to replace behaviours of concern (skill development)
  • Provide consistent responses when behaviours occur (reactive strategies)
  • Promote the person’s independence, safety, and quality of life through supportive, person-centred approaches (wellbeing and inclusion)

Behaviour Support Training

The main goal of Behaviour Support Training is to:

  • Build staff and caregiver understanding of the person’s behaviour and support needs (knowledge development)
  • Ensure consistent use of strategies outlined in the Behaviour Support Plan (practice consistency)
  • Strengthen skills in proactive, positive, and least restrictive approaches (capacity building)
  • Promote confidence, collaboration, and quality of care across all support environments (team effectiveness)

Ongoing Review and Evaluation

The main goal of Ongoing Monitoring, Review and Evaluation is to:

  • Track the person’s progress and response to strategies over time (data collection and observation)
  • Assess the effectiveness of interventions and identify areas for improvement (evaluation)
  • Update goals, strategies, and supports as the person’s needs or circumstances change (review and adjustment)
  • Ensure continuous alignment with best practice and promote positive, sustainable outcomes (quality improvement)
